This will include a lab authorization to have the following labs completed;
Lipids
CMP
TSH
A1c
Patients will need to bring the authorization and a valid photo ID to the nearest clinical lab / collection location (appointment may be required).
Locations will be available to patient based on your zip code.
Fasting is preferred for Lipid results, but not required.
